Alera Group, Inc. (Case No. CACE25019102, Circuit Court for Broward County, Florida). Submit online or by mail postmarked by June 29, 2026. All class members may enroll in two years of free CyEx Medical Shield financial data monitoring and claim one cash payment: Cash Payment A reimburses documented out-of-pocket losses up to $3,500 (requires proof), or Cash Payment B is a flat alternate payment estimated at $50 with no proof required. Cash payments are pro rata, subject to a $2,000,000 cap.
