The class covered everyone residing in the United States whose personal or protected health information was compromised in the incident. The case was Skov et al. v. Lakeview Health Systems, L.L.C., No. 16-2024-CA-003154-AXXX-MA, in the Circuit Court of Duval County, Florida.
What you can claim
- One year of CyEx Medical Shield Total credit monitoring, open to every class member.
- Ordinary losses of up to $2,000 with proof.
- Extraordinary losses from identity theft or fraud of up to $5,000 with proof.
- Lost time of up to $80, covering four hours at $20 an hour, with proof.
- A flat alternative cash payment of $50 with no proof required.
How payment works

What could class members claim?
Every class member could enroll in one year of CyEx Medical Shield Total credit monitoring. On top of that they could either document their losses, ordinary losses up to $2,000, extraordinary losses from identity theft or fraud up to $5,000, and lost time up to $80 for four hours at $20 an hour, or take a flat alternative cash payment of $50 with no documentation. The two routes could not be combined.
Was proof required?
It depended on the route. The documented benefits all required proof: receipts, statements or other records showing the loss and its connection to the incident. The flat $50 alternative cash payment required no documentation at all, which is why it was capped so much lower than the documented amounts.
Can I still file a claim?
No. Claims had to be submitted online, by mail or by email and postmarked by August 24, 2026. That deadline has passed. This page remains as a record of what the settlement covered. Claims filed before the deadline are still processed by the settlement administrator on its own schedule.
