All individuals residing in the United States whose PII/PHI was compromised in the Lakeview Health data incident discovered on or around January 25, 2024. Skov et al. v. Lakeview Health Systems, L.L.C. (Case No. 16-2024-CA-003154-AXXX-MA, Circuit Court of Duval County, Florida). Submit online, by mail, or by email postmarked by August 24, 2026. All class members may enroll in one year of free CyEx Medical Shield Total credit monitoring and claim cash benefits: Ordinary Losses up to $2,000, Extraordinary Losses (identity theft/fraud) up to $5,000, and Lost Time up to $80 (4 hours at $20/hr) — all require proof.
